Corrective and Preventive Action (CAPA)

Corrective and preventive action is a quality management process for investigating non-conformities and other problems, eliminating their causes to prevent recurrence, and addressing the causes of potential problems before they occur.

A corrective action addresses the cause of an existing non-conformity, such as a defect, customer complaint, audit finding or process deviation, so that it does not happen again. A preventive action addresses the cause of a potential non-conformity identified, for example, from trend data or risk analysis. A typical CAPA process records the problem, assesses its risk, takes immediate containment or correction, performs root cause analysis, defines and implements actions, verifies their effectiveness and closes the record with evidence.

CAPA is a central element of regulated quality systems. ISO 13485 for medical devices contains separate requirements for corrective and preventive action, and the ICH Q10 pharmaceutical quality system guideline names a CAPA system as one of its key elements. ISO 9001 requires corrective action and, since its 2015 revision, addresses prevention through risk-based thinking rather than a separate preventive action clause. Automotive suppliers often document corrective actions in 8D reports.

CAPA systems become overloaded when every minor issue is raised as a CAPA, so many organisations use risk-based triage. Closing actions without verifying effectiveness, or defaulting to retraining as the action, are common audit findings. Useful measures include CAPA cycle time, overdue actions and the recurrence rate of problems that were supposedly corrected.
