8D Problem Solving (Eight Disciplines)
8D is a structured, team-based problem-solving method, widely used in the automotive industry, that moves through eight disciplines from forming a team and containing the problem to finding root causes, correcting them and preventing recurrence.
The method was popularised by Ford Motor Company and is commonly described as follows: D1, form a cross-functional team; D2, describe the problem in measurable terms; D3, implement interim containment actions to protect the customer; D4, identify and verify root causes, including why the problem escaped detection; D5, choose and verify permanent corrective actions; D6, implement and validate them; D7, prevent recurrence by updating systems, standards and similar processes; and D8, recognise the team. Many versions add D0, planning and emergency response.
8D is used for significant quality problems, especially customer complaints and supplier non-conformances, and many automotive and industrial customers require suppliers to respond in an 8D report format. It combines root cause tools such as 5 whys, fishbone diagrams and is and is-not analysis with containment and verification steps, linking problem-solving to the corrective action process of a quality management system.
An 8D is only as good as its root cause verification; reports that list assumed causes and generic actions such as retraining rarely prevent recurrence. Containment should take effect quickly, while permanent corrective actions are verified before they replace containment. Updating FMEAs, control plans and work instructions in D7 helps prevent the same failure elsewhere.
