Poka-Yoke
Poka-yoke is a lean technique for error-proofing processes so that mistakes cannot be made, or are detected immediately before they become defects, using simple physical, sensing or procedural devices.
The term, Japanese for mistake-proofing, was popularised by Shigeo Shingo as part of his zero quality control approach. Poka-yoke devices work in two ways. Prevention devices make an error impossible, for example asymmetric connectors or fixtures that accept a part only in the correct orientation. Detection devices sense an error and stop the process or warn the operator, for example a check that all fasteners have been tightened before a part can be released. Methods are often classed as contact, fixed-value and motion-step methods.
Poka-yoke is used throughout assembly, machining, packaging and logistics to prevent missing parts, wrong parts, incorrect orientation and skipped steps. It reduces reliance on inspection and operator vigilance, and is a typical countermeasure in root cause analysis and corrective action. In process FMEA, error-proofing can lower the occurrence rating or improve the detection rating of a failure mode.
Effective poka-yoke devices are simple, inexpensive and placed close to where errors occur. Their function should be verified regularly, for example with known bad parts at the start of each shift, because a failed device gives false confidence. Error-proofing for quality is distinct from machine safeguarding: safety functions must be designed to the applicable functional safety and machinery standards.
