Fault Tree Analysis (FTA)
Fault tree analysis is a top-down, deductive method that models how combinations of component failures, human errors and external events can lead to a specified undesired top event, using logic gates such as AND and OR.
An analyst starts with a top event, such as loss of cooling or an unintended machine start, and works downwards to identify the immediate causes and how they combine. An OR gate means any one input causes the output; an AND gate means all inputs must occur together. The tree continues down to basic events, such as individual component failures, whose probabilities or failure rates can be estimated. Minimal cut sets, the smallest combinations of basic events that cause the top event, show the system's weak points, and single-event cut sets reveal single points of failure.
FTA was developed at Bell Telephone Laboratories in the early 1960s for the Minuteman missile launch control system and is now used in aerospace, nuclear, chemical process, rail, automotive and machinery safety. It supports design reviews, safety cases, functional safety assessments and root cause investigations, and complements bottom-up methods such as FMEA and FMECA.
Quantitative FTA needs reliable failure data and careful treatment of common-cause failures, which can defeat redundancy that the tree would otherwise treat as independent. Large trees are built and solved with software, and the quality of the analysis depends on the analyst's understanding of the system. IEC 61025 describes fault tree analysis.
